Sample Starter Kit Order Form Filling Steps
  1. Fill in Customer Name
  2. Fill in Ship to Address-you may want samples sent to your customer or to an architect who wants to see the materials before they specify Smart Cabinetry in a home or building.
    Smart Cabinetry will do that for you as long as the forms are filled out properly.
  3. Fill in Bill to Address
  4. Fill in Date when order placed-Smart Cabinetry uses this information to report on ourselves.
    We have an extensive manufacturing reporting system that grades our manufacturing team on service levels.
    If you have information on this form it contributes to the well being of Smart and the service levels you receive.
  5. Include P.O. # (Purchase Order #).
    All orders received by Smart must include purchase order # if they are to be considered legitimate.
    Any and all orders without purchase order will be sent back for appropriate information before they are entered and processed.
  6. Sent By We need to know who sent in the request just in case we need to ask a question about the order.
  7. Fill out order form by placing the QTY of the item requested and fax or email to Smart Cabinetry.
